Abstract
Background: Biologic therapies, such as omalizumab, mepolizumab, dupilumab, and benralizumab, are increasingly used in the treatment of various type 2 inflammatory diseases, including chronic rhinosinusitis with nasal polyposis (CRSwNP), asthma, and atopic dermatitis. While these biologic therapies have demonstrated efficacy and safety in their respective clinical trials, real-world characterization of the use of biologic therapies provides insight into how these treatments are prescribed and utilized among the general population.
Objective: We aim to characterize the patient population in a single tertiary otolaryngology practice that is receiving biologic therapies for all type 2 inflammatory diseases.
Methods: We conducted a retrospective chart review to identify patients prescribed a biologic agent between January 2015 and December 2022. Collected data included demographic information and clinical progress after the initiation of biologic therapy. The primary measurement of clinical progress consisted of changes in the use of systemic corticosteroids and the need for sinonasal surgery during the periods 2 years before and after biologic initiation.
Results: From an initial 110 patients identified as receiving biologic therapies, 100 patients provided sufficient clinical follow-up and were included for analysis. Sixty-five of the patients were female. The mean age was 50 (range 9-82). There was a significant decrease in the median scores from the Sinonasal Outcome Test (SNOT-22) before and after initiation of a biologic agent (41.5 and 15.3, p0.001).
Conclusion: Our data show that biologic therapies in rhinologic practice play an effective role in the management options of various inflammatory conditions, including CRSwNP, asthma, and atopic dermatitis. For CRSwNP, biologics improve sinonasal symptoms and, in a real-world setting, reduce the need for oral corticosteroids and surgical interventions.
